How pay per head works
Pay per head is a service model. A bookmaker rents a complete betting operation, meaning the website, the odds, the casino, the reporting and the customer support behind it, and pays a weekly fee for every player who actually places a bet. The bookmaker keeps the win, covers the loss, and pays his own players.

Odds, the betting site, the casino, the apps, the weekly reporting, and the people answering the phone at 1am when a player can't log in.
$8 on Standard, $11 on Pro. A player counts as active in a week only if he places a wager. A player who logs in and doesn't bet costs you nothing.
